Affiliate Program
The Affiliate Program is designed for institutions that are currently not registered and/or licensed with the Food and Drug Administration; and organizations, international facilities and professional groups in the health-care profession.
The fee for Affiliates is $1,050 per calendar year (January 1 to December 31). To join the Affiliate Program, download an application (PDF) and fax or mail it to:
