Supporting Documentation for Memorial Award Nomination Submissions
Please include the following information on the
Online Nomination Form when submitting Memorial Award nominations:
- A thorough description of the nominee’s responsibilities; and
- A summary of significant contribution(s) and/or achievement(s) relating to the specific Award criteria for which the individual is being nominated and judged.
- Important information from the nominee’s curriculum vitae or biography is welcome.
Materials submitted will not be returned.
