How to Submit a Nomination

​A nomination form must be submitted for each nomination submission.

Complete the Online Nomination Form by providing all supporting documentation requested. (See below for more details.)

All nomination forms must be received by Friday, Jan. 27, 2017.

Supporting Documentation for Memorial Award Nomination Submissions

Please include the following information on the Online Nomination Form when submitting Memorial Award nominations:

  • A thorough description of the nominee’s responsibilities; and
  • A summary of significant contribution(s) and/or achievement(s) relating to the specific Award criteria for which the individual is being nominated and judged. 
  • Important information from the nominee’s curriculum vitae or biography is welcome.

Materials submitted will not be returned.
