This edition of Perioperative Standards, effective January 1, 2027, contains requirements that address perioperative collection and preparation of red cell and non-red cell components. Its goal is to maintain and enhance the quality and safety of perioperative autologous blood collection and administration and to provide a basis for the AABB Accreditation Program. It reflects the concerns and priorities of several different aspects of the discipline, including the input of recognized experts in the field and the best interests of their patients. Addition to ensure that clear communication exists between AABB and all accredited laboratories in cases where an event has occurred that has impacted their accreditation status.
Addition to ensure that third-party providers of perioperative services have staff who are trained and competent to meet the requirements in the 12th edition.
Addition requiring all perioperative programs to review and manage the workload of all personnel to ensure patient safety.
Addition to ensure that all laboratories monitor their critical technology infrastructure and that they function as expected. There shall be defined checks to monitor that technology is working as intended and expected.
All blood and blood components must be labeled clearly and identified for perioperative administration or reinfusion.
Programs using alternative physiological wash fluid must have policies, processes, and procedures for its use.
Programs using normothermic machine perfusion for organ procurement must have policies, processes, and procedures for the use.

Guidance for Standards for Perioperative Autologous Blood Collection and Administration includes verbatim requirements from the Perioperative Standards interspersed with guidance. Language from the Perioperative Standards is presented; when there is appropriate guidance for that standard, the guidance language follows the standard with the heading "Guidance."" This format allows the reader to see both the requirement and the associated guidance side by side.
