Australia’s Blood Matters Program, a joint initiative of the Victoria state government and Australian Red Cross Lifeblood, has
published the 2022–23 Serious Transfusion Incident Reporting (STIR) report. The report includes 209 validated investigations, including 68 procedural events and 141 clinical reactions. Preventable procedural events accounted for one-third of all reported incidents.
The report features detailed case studies to help clinicians and health services identify risks, evaluate current practices, and implement targeted improvements. Health care professionals can access the full report
